How to create a survey/form

Easily create a survey using the forms function

1. Navigate to a space and click on Files

2. Click on the plus sign and select “create form”

3. Click on the edit button (little pencil) and give your form a name

4. Choose a preset template OR start from scratch and add a first question. Choose the right format amongst the following options:

  • Paragraph (text)
  • Numeric
  • Email
  • List (Multiple Choices (Single answer) or Checkboxes (Multiple answers))
  • Dates
  • Phone Number
  • Links
  • Description (to provides context for your questions and guiding your respondents in understanding what is expected of them)
  • Image 


5. You'll find several options for each question you add: Duplicate, Delete, and the option to make a question Required.

6. Once you have all your questions, navigate to the Settings button to select the options that best suit your form:

  • Limit to 1 response per user
  • Allow response editing: by enabling this option, you allow respondents to modify or update their responses even after they have submitted their answers.
  • Make responses anonymous: activate this option for sensitive topics when anonymity is important.

 7. Once you're happy with it and ready to make it public, click on Draft in the upper right hand corner.

8. If you want it to be published on the homepage and everyone to be notified, click on Add announcement, if not, simply click on Publish.
